Yes is required for:

initial certifications,
recertification for those who have not administered STC in 2 or more years, or
in any case that the leadership team assesses that an individual with prior certification would benefit from participating in a preceptorship and submission of a Cancer Systemic Therapy Certification course skills assessment checklist is warranted
